What are the possible side effects of Quinoctal?
Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
Stop using Quinoctal and call your doctor at once if you have any of these serious side effects:
- fever, chills, confusion, weakness, sweating;
- severe vomiting, stomach pain, diarrhea;
- problems with vision or hearing;
- chest pain, trouble breathing, severe dizziness, fainting, fast or pounding heartbeats;
- severe flushing (warmth, redness, or tingly feeling);
- urinating less than usual or not at all;
- weak or shallow breathing, feeling like you might pass out;
- easy bruising, unusual bleeding (nose, mouth, vagina, or rectum), purple or red pinpoint spots under your skin;
- blood in your urine or stools;
- fever, sore throat, and headache with a severe blistering, peeling, and red skin rash; or
- loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes).
Less serious side effects may include:
- headache, blurred vision, changes in color vision;
- mild dizziness, spinning sensation, ringing in your ears;
- upset stomach; or
- muscle weakness.

Side effects of Quinoctal in details
Overall
Quinoctal can adversely affect almost every body system. The most common adverse events associated with Quinoctal use are a cluster of symptoms called "cinchonism", which occurs to some degree in almost all patients taking Quinoctal. Symptoms of mild cinchonism include headache, vasodilation and sweating, nausea, tinnitus, hearing impairment, vertigo or dizziness, blurred vision, and disturbance in color perception. More severe symptoms of cinchonism are vomiting, diarrhea, abdominal pain, deafness, blindness, and disturbances in cardiac rhythm or conduction. Most symptoms of cinchonism are reversible and resolve with discontinuation of Quinoctal.
The following ADVERSE REACTIONS have been reported with Quinoctal. Because these reactions have been reported voluntarily from a population of uncertain size, it is not always possible to reliably estimate their frequency or establish a causal relationship to drug exposure.
General: fever, chills, sweating, flushing, asthenia, lupus-like syndrome, and hypersensitivity reactions.
Hematologic: agranulocytosis, hypoprothrombinemia, thrombocytopenia, disseminated intravascular coagulation, hemolytic anemia; hemolytic uremic syndrome, thrombotic thrombocytopenic purpura, idiopathic thrombocytopenic purpura, petechiae, ecchymosis, hemorrhage, coagulopathy, blackwater fever, leukopenia, neutropenia, pancytopenia, aplastic anemia, and lupus anticoagulant.
Neuropsychiatric: headache, diplopia, confusion, altered mental status, seizures, coma, disorientation, tremors, restlessness, ataxia, acute dystonic reaction, aphasia, and suicide.
Dermatologic: cutaneous rashes, including urticarial, papular, or scarlatinal rashes, pruritus, bullous dermatitis, exfoliative dermatitis, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, fixed drug eruption, photosensitivity reactions, allergic contact dermatitis, acral necrosis, and cutaneous vasculitis.
Respiratory: asthma, dyspnea, pulmonary edema.
Cardiovascular: chest pain, vasodilatation, hypotension, postural hypotension, tachycardia, bradycardia, palpitations, syncope, atrioventricular block, atrial fibrillation, irregular rhythm, unifocal premature ventricular contractions, nodal escape beats, U waves, QT prolongation, ventricular fibrillation, ventricular tachycardia, torsades de pointes, and cardiac arrest.
Gastrointestinal: nausea, vomiting, diarrhea, abdominal pain, gastric irritation, and esophagitis.
Hepatobiliary: granulomatous hepatitis, hepatitis, jaundice, and abnormal liver function tests.
Metabolic: hypoglycemia and anorexia.
Musculoskeletal: myalgias and muscle weakness.
Renal: hemoglobinuria, renal failure, renal impairment, and acute interstitial nephritis.
Special Senses: visual disturbances, including blurred vision with scotomata, sudden loss of vision, photophobia, diplopia, night blindness, diminished visual fields, fixed pupillary dilatation, disturbed color vision, optic neuritis, blindness, vertigo, tinnitus, hearing impairment, and deafness.
What is the most important information I should know about Quinoctal?
- Quinoctal may cause dizziness, light-headedness, or blurred vision. These effects may be worse if you take it with alcohol or certain medicines. Use Quinoctal with caution. Do not drive or perform other possibly unsafe tasks until you know how you react to it.
- Tell your doctor or dentist that you take Quinoctal before you receive any medical or dental care, emergency care, or surgery.
- If your symptoms do not get better within 1 to 2 days or if they get worse, or if your fever comes back after finishing treatment with Quinoctal, check with your doctor.
- Quinoctal may cause low blood sugar. Pregnant women may be at higher risk of low blood sugar. Low blood sugar may make you anxious, sweaty, weak, dizzy, drowsy, or faint. It may also make your heart beat faster; make your vision change; give you a headache, chills, or tremors; or make you more hungry. It is a good idea to carry a reliable source of glucose (eg, tablets or gel) to treat low blood sugar. If this is not available, you should eat or drink a quick source of sugar like table sugar, honey, candy, orange juice, or non-diet soda. This will raise your blood sugar level quickly. Tell your doctor right away if this happens.
- Do NOT take more than the recommended dose or use for longer than prescribed without checking with your doctor.
- Quinoctal may reduce the number of clot-forming cells (platelets) in your blood. Avoid activities that may cause bruising or injury.
- Serious and life-threatening bleeding problems may occur with Quinoctal. This could also lead to severe kidney problems. Tell your doctor right away if you notice symptoms of unusual bleeding or bruising (eg, bleeding gums; severe nose bleed; dark urine; black, tarry, or bloody stools; unusual purple, brown, or red spots on the skin).
- Quinoctal should NOT be used to treat or prevent nighttime leg cramps. It should also NOT be used to prevent malaria. Talk with your doctor about other ways to treat nighttime leg cramps or to prevent malaria.
- Quinoctal may interfere with certain lab tests. Be sure your doctor and lab personnel know you are taking Quinoctal.
- Use Quinoctal with caution in the ELDERLY; they may be more sensitive to its effects.
- Quinoctal should be used with extreme caution in CHILDREN younger than 16 years; safety and effectiveness in these children have not been confirmed.
- PREGNANCY and BREAST-FEEDING: If you become pregnant, contact your doctor. You will need to discuss the benefits and risks of taking Quinoctal while you are pregnant. Quinoctal is found in breast milk. If you are or will be breast-feeding while you take Quinoctal, check with your doctor. Discuss any possible risks to your baby.
Quinoctal contraindications
Quinoctal Capsules are contraindicated in patients with the following:

- Prolonged QT interval. One case of a fatal ventricular arrhythmia was reported in an elderly patient with a prolonged QT interval at baseline, who received Quinoctal intravenously for P. falciparum malaria.
- •
- Glucose-6-phosphate dehydrogenase (G6PD) deficiency.
- •
- Hemolysis can occur in patients with G6PD deficiency receiving Quinoctal.
- •
- Known hypersensitivity reactions to Quinoctal.
- •
- These include, but are not limited to, the following :
- •
- Thrombocytopenia
- •
- Idiopathic thrombocytopenia purpura (ITP) and Thrombotic thrombocytopenic purpura (TTP)
- •
- Hemolytic uremic syndrome (HUS)
- •
- Blackwater fever (acute intravascular hemolysis, hemoglobinuria, and hemoglobinemia)
- •
- Known hypersensitivity to mefloquine or quinidine: cross-sensitivity to Quinoctal has been documented.
- •
- Myasthenia gravis. Quinoctal has neuromuscular blocking activity, and may exacerbate muscle weakness.
- •
- Optic neuritis. Quinoctal may exacerbate active optic neuritis.
